Food Diary

Please complete a 3-day food diary before your initial consultation.

The food diary is so I can have a look at your current eating habits. Please choose three days that reflect your normal, daily eating habits. If you’ve had a one-off meal that isn’t your norm, you don’t need to put it down.

  • Try to record information as soon as possible to get the most accuracy
  • Please include details of the type of food, e.g. white bread, wholegrain, full fat, fat free, sugar free, dairy free spreads, butter or margarine etc…
  • Where applicable please state the amount of food e.g. 1 teaspoon of sugar
  • Please note any feelings you may notice after certain foods e.g. – cramping after pasta
  • Please also state the time of your meals e.g. breakfast 8am, snacks and drinks 11pm
  • In additional comments, explain why you may have skipped a meal, if you didn’t have time to cook, if the food is freshly made, restaurant food, ready meal, if you ate while working at your desk etc…

How to hand in your food diary

  • You can collect this information in any way that’s convenient to you and send it to me before your initial consultation.
  • A PDF you can print and fill in by hand is provided below.
  • Feel free, though, to just write the same information on any piece of paper.
  • Please note the form below does not have a “Save” functionality, meaning you are not able to save your progress and come back to it. You will need to enter and submit your information all at once.
